Chronicle Reporting Capabilities

  • Delta Report(link is external) (Available to Minimal Capacity Users) 
    • Generates aggregate behavioral change outcomes 
    • Generates aggregate participant outcomes 
    • Generates additional biometrics
  • ERP Site Report (link is external)(Available to Minimal Capacity Users) 
    • Generates the total number of participants during reporting period 
    • Generates the average hours of DSME received by participants during reporting period 
    • Generates the demographics of participants seen during the reporting period
  • Educator Hours Report(link is external) 
    • Generates Summary of educator Hours 
  • Filter Report(link is external) (Available to Minimal Capacity Users) 
    • Generates aggregate reports by numerous filters which now include provider and insurance type for the purpose of advertising program outcomes to insurers and providers.
    • Generates individual participant reports which can include participant demographics as well as initial and most recent behavior objectives and lab data
  • Lost to Follow-up Report(link is external) 
    • Provides the number of participants that have behavioral goals set with no follow-up
  • No Show Report(link is external) 
    • Provides the contact information for participants that have been placed in No Show status 
  • Hospital & ER Visit Report(link is external) (Available to Minimal Capacity Users) 
    • Generates the  % change in hospital & ER visits post DSMES, both due to diabetes and not due to diabetes. 

High-Level Report

This report can be downloaded as a standalone report, or it can be exported through the filter report which allows you to filter report data based on provider, demographics, clinical lab information, and numerous other filters chosen by the educator.
